Found a Lost Badge or RebelCard?

Should you find a lost hospital badge or RebelCard, please return it to the front desk of the office of student affairs and admissions in MEB Room 303. The school of medicine office of space and facilities will be notified to deactivate the card, and the owner will be informed.

Hospital Badges

University Medical Center Southern Nevada (UMC) Hospital

Please contact Ericka Shipley, clinical education coordinator, at ericka.shipley@unlv.edu, before visiting the UMC badging office.

For All Other Local Hospitals

For all other local hospitals and/or clinics, including the VA Southern Nevada Healthcare System, UNLV Health, the Southern Nevada Health District, etc., please contact Ericka Shipley at ericka.shipley@unlv.edu. Replacement pickup will be on the hospital premises.

RebelCards

Please visit the RebelCard Online Office and suspend activity on your account.

Next, send an email to the RebelCard Office at rebelcard@unlv.edu and facilities@medicine.unlv.edu indicating that you are a school of medicine student and need a replacement card issued. Your lost card will be deactivated and your replacement card will be added to your account. 

Please be aware that there will be a $25 replacement fee, which can be paid online in your RebelCard Account. The expected processing time is two to three days.

Be sure to monitor your email for a notification from the office of student affairs confirming that your replacement card is ready for pickup. The new card will be available for collection at the front desk of the office of student affairs, located in MEB Room 303.

RebelCard Loaner

If you need to check out a RebelCard loaner while you wait for your replacement card, please visit the office of student affairs in MEB Room 303. Please note that the loaner card must be returned upon pickup of your replacement RebelCard.
