Guidelines for Submitting Documents

Before uploading documents in the Rebel Success Hub:

  • Verify all documents are clear, legible, and complete.
  • Use blue or black ink on all documents.
  • Verify each paper submitted contains the student’s name and NSHE ID.
  • Verify that the IRS taxes are signed.
  • If a signature is required, the signature must be hand-signed.

Satisfactory Academic Progress (SAP) Appeal

Complete the form specifically for the criteria you are not meeting to appeal your Satisfactory Academic Progress status.

Scholarship Appeal

Complete this form if you would like to appeal the cancellation of a scholarship.

Special Circumstances Appeals

Complete this appeal to have your financial aid file reviewed if there has been a significant change in your situation since filing the Free Application for Federal Student Aid (FAFSA).

Students who are submitting a Special Circumstance, Unusual Circumstance and/or a Cost of Attendance Appeal are encouraged to do so by the review dates specified below to ensure review is complete by the Friday before classes begin. Appeals will still be considered after these dates, but review might not be complete until after classes have begun:

Unusual Enrollment History Appeal Form

Unusual Enrollment History (UEH) is the specific pattern the U.S. Department of Education uses to select students for further review who have received federal aid at multiple institutions during the last four academic years. Students are required to have earned academic credit during the last four years in which they received federal aid such as Federal Pell and/or Federal Direct Loans. The Office of Financial Aid & Scholarships understands that students may have legitimate reasons for their enrollment at multiple institutions. Our office will review students who have the "UEH flag" on their FAFSA. If an appeal is needed, a case will be created in the Rebel Success Hub. If there are any questions about this process, students can open a case in the Rebel Success Hub.

Federal Aid Change Form

Complete this form if you are requesting additional student loan eligibility, previously declined your financial aid, canceled previously accepted financial aid, or changed your housing arrangements (On-Campus, Off-Campus, With Parents).

Federal Loan Proration Form

Complete this form only if you are an undergraduate student and will be enrolled in one semester of the academic year (Fall, Spring, or Summer).

Loan Discharge Reaffirmation Form

Complete this form if you previously had a student loan discharged because of a total permanent disability and you want to borrow additional federal student loans.

Fall Semester Loan Adjustment Form

Complete this form if you would like your federal student loans adjusted to one semester up to maximum borrowing eligibility.

Parent PLUS Loan Replacement Form

Complete this form if the parent PLUS loan was denied and the student desires to borrow additional funds under the unsubsidized loan program. Graduate students are not eligible to complete this form.

Non-Federal Private Loan Forms

Complete this form if you are applying for a non-federal private student loan, or you would like to make changes to the loan amount of a previously approved private student loan.

Alternate Need Determination Form

Complete this form by logging onto Self-Service to determine your financial need for select UNLV scholarship and grant programs as a non-FAFSA eligible student. Do not complete this form if you are a U.S. Citizen or an eligible non-citizen.

Complete this form if you filled out your FAFSA application, and the U.S. Department of Education could not confirm your citizenship status. This form is to be completed if you cannot present the required citizenship documents to the UNLV Financial Aid & Scholarships Office. Note: Until further notice, the notary section of the form is waived due to the COVID-19 pandemic.

TEACH Grant by the U.S. Department of Education

Millennium Scholarship Co-Enrollment Form

Complete this form if you are a Millennium Scholarship student and you wish to take a portion of your credits at another Nevada System of Higher Education (NSHE) college or university.

Millennium Scholarship Study Abroad Partnership Form

Complete this form if you are participating in a transfer credit (partnership) program to request matching grant funds in place of the Millennium Scholarship.

Millennium Summer Reimbursement Form

Complete this form if you are a Millennium Scholarship student, but you are not degree-seeking at the eligible institution you are enrolling in summer classes.

Millennium Students with Documented Disabilities Form

Complete this form if you are a student with a documented physical or mental disability or who were previously subject to an individualized education program and are requesting to enroll with Millennium Scholarship support in fewer than the minimum semester credit hours or an extension of the expiration date for funding.

Summer 2025 forms will be available beginning March 2025.

Asset Form

Complete the 2024-25 Asset Worksheet if you are asked to clarify your asset information from the FAFSA you submitted.

IRA/Pension Rollover Worksheet

Please use this form if someone in your household received an IRA, pension plan, or annuity distribution that included a Rollover. While traditional distributions from an IRA, pension plan, or annuity are considered untaxed income on the FAFSA, qualified Rollovers do not have to be reported.

Verification Income Information for Individuals with Unusual Circumstances

If you were selected for verification and you/your parent(s) have an unusual circumstance preventing you from obtaining the required annual income tax data, please see the document below for guidance. An unusual circumstance would be if you/your parent(s) were granted an extension, filed an amended tax return, are the victim(s) of tax identity theft, or filed a non-IRS income tax return.
