There is an ongoing issue with committee forms in UNLV Folio only displaying the 'Submit Form' and 'Save Responses' button - none of the other form fields are being displayed. At this time, there are two workarounds you can try. If these workarounds do not work for you, please contact unlvfolio@unlv.edu to troubleshoot further. 

1. Access the form from a different computer.

2. Access the form from an incognito browser window on Google Chrome.
